register before giving birth. You can register in various ways.
Choose the method that is most suitable for you and complete the
registration process as soon as possible.

order to receive a registration form via email or fax or any
other information regarding registration for birth, please
contact: 02-646-4262, 24 hours a day or write to us via
email.
If you chose
to download a registration form from the internet, please follow
these instructions:
• Save the form in your
computer.
• Open the document and fill in all the
details on the computer, or print the registration form out and
fill in the details by hand.
• You can send the
